Amy Harant is considered an in-network preferred providers for some of Blue Cross Blue Shield’s private plans which include Premier BlueCard PPO, Highmark, Horizon, and Anthem.
We are considered out-of-network providers through all other insurance plans. Payments are due upfront and if your claim is accepted a check will be sent to you from your insurance company. We also submit detailed invoices to all of our families. You may submit this invoice to your insurance company for out-of-network reimbursement. You will need to call your insurance company to find out if you have out-of-network benefits, what is your out-of-network deductible, and what is your out-of-network yearly limit.
“All benefits are not a guarantee of payment; all claims are subject to review and must meet the medical criteria of their plan.”
